Qualifications

• Current Kentucky Physical Therapist (PT) license required.
• Graduate of an accredited Physical Therapy program.
• Experience in rehabilitation, neurological rehabilitation, long-term care, or healthcare settings preferred.
• Strong assessment, treatment planning, and clinical skills.

Responsibilities

• Conduct physical therapy evaluations and develop individualized treatment plans.
• Provide skilled therapy services based on functional goals and clinical needs.
• Monitor progress and modify treatment interventions as appropriate.
• Promote mobility, safety, and independence in daily activities.
• Communicate changes in condition, safety concerns, or barriers to progress.
• Collaborate with physicians, nursing staff, case managers, and other interdisciplinary team members.
• Complete all documentation accurately and within required timeframes.
• Ensure documentation supports medical necessity, compliance standards, and billing requirements.
• Maintain accurate records of all services provided and hours worked.
• Participate in care planning and interdisciplinary communication as needed.
• Perform other duties as assigned.
