Job summary

Eastgate Nursing & Rehabilitation Center is seeking a licensed Physical Therapist to provide and direct therapy services to adult and geriatric patients in a skilled nursing setting. The role includes overseeing support personnel and collaborating with the Interdisciplinary Team (IDT) to ensure compliant care.

Responsibilities include performing assessments, developing and implementing treatment plans, coordinating orders, and monitoring progress for optimal patient outcomes.

Qualifications

• Current RI PT license required.
• Ability to oversee and guide support personnel.
• Desire to work in a collaborative environment with the IDT.
• Knowledge of state and federal regulations and billing practices.

Responsibilities

• Provides physical therapy services to patients and initiates referrals when appropriate.
• Conducts screenings of residents at regular intervals to determine need for interventions/treatment.
• Evaluates residents to obtain data for treatment planning and implementation; conducts specialized evaluations as indicated.
• Obtains physicians’ orders/documentation for treatment.
• Interprets and communicates evaluation findings to patients, families, and other team members.
• Develops treatment plans with short- and long-term goals and methods to achieve them.
• Coordinates, implements, and conducts treatment and plans with patients, family, and other team members.
• Monitors patient responses to intervention and modifies treatment as indicated to achieve goals.
• Willingness to work with severely physically, emotionally and cognitively compromised patients.
