Job summary

Golfcrest Nursing and Rehabilitation Center seeks a qualified Physical Therapy leader to oversee the delivery of services for residents. The role ensures compliance with regulatory standards and physician orders while guiding the PT team and coordinating care across the facility.

The incumbent will monitor performance indicators, maintain documentation, and foster positive relationships with residents, staff, and families, ensuring compassionate, high-quality therapy services.

Qualifications

• A valid, unrestricted license in his/her therapy discipline in the state.
• Meet educational requirements of his/her licensure requirements.
• Knowledgeable of skilled nursing home regulations, procedures, laws, regulations and guidelines pertaining to long-term care.

Responsibilities

• Plans, develops, organizes, implements, evaluates and directs the delivery of physical therapy services in accordance with state and federal laws and facility policies.
• Coordinates execution of physical therapy services per physician orders and APTA guidelines.
• Monitors facility key performance indicators for physical therapy and reports outcomes to the rehabilitation director.
• Develops and implements processes to lead and direct PT services, including screening, evaluation, treatment, discharge and documentation.
• Provides resident information for assessments and maintains communication with residents, families, and staff.
• Evaluates PT assistants and maintains adherence to standards of practice and code of conduct.
