Job summary

Eastern Health Group seeks a licensed Physical Therapist to join its resident-centered rehabilitation team. You will assess referred patients, develop treatment plans, and work under physician supervision to help patients regain function.

Experience in long-term care or geriatric settings is preferred; licensure must be valid in the state. We offer a positive environment, thorough training, competitive pay, and benefits.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care to assess medical condition, function, and rehab potential.
• Establish and administer a treatment program under Physician direction.
• Set realistic goals for patients and document progress and outcomes.
• Educate patients and families in physical therapy methods.
• Complete evaluations, weekly progress notes, recertifications and discharge documents.
• Collaborate with all disciplines to deliver patient-centered care.
• Perform duties assigned by Director of Rehabilitation.
• Maintain confidentiality and HIPAA compliance.
