Job summary

Tender Touch Rehab Services is seeking a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team, providing individualized therapy to seniors in skilled nursing and long-term care settings. This on-site role focuses on mobility, function, safety, and quality of life, with collaboration across an interdisciplinary team.

You will implement PT treatment plans, document progress in EMR, and educate patients and caregivers, while benefiting from mentorship

Qualifications

• Current licensure as PTA in the state where practicing.
• Experience with geriatric populations preferred.
• Strong communication and compassionate patient care.

Responsibilities

• Implement PT treatment plans to improve mobility, function, safety and quality of life.
• Perform therapeutic exercises, gait and balance training, and functional activities.
• Accurately document patient progress in EMR.
• Educate patients and caregivers to support ongoing care.
