Tender Touch Rehab Services in Pennsylvania is seeking a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team, providing individualized therapy to seniors in skilled nursing and long-term care settings. This on-site role focuses on mobility, function, safety, and quality of life.

You will implement PT treatment plans, lead therapeutic exercises, gait and balance training, document accurately in the EMR, and educate patients and caregivers.

Qualifications

• Must be Certified Physical Therapist Assistant with active licensure in PA.
• Provide individualized therapy to seniors in skilled nursing and long-term care settings.
• Implement PT treatment plans focusing on mobility, function, safety and quality of life.

Responsibilities

• Deliver therapeutic exercise, gait and balance training, and functional activities.
• Accurately document care in the EMR system.
• Educate patients and caregivers on treatment plans and home programs.
• Collaborate with an interdisciplinary team to support patient-centered care.
