Responsibilities

• Evaluates and implements physical therapy treatment plans.
• Collaborates with physician, family and rehab team to coordinate care.
• Documents treatment plans and progress according to standards.
• Provides education to family/caregivers and other professionals.
• Demonstrates flexibility to work across inpatient, outpatient, and PACE as needed.
