Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a Physical Therapist for Wilson Assisted Living. This full-time role offers a sign-on bonus and a comprehensive benefits package, including medical/dental/vision and a 401K.

You will evaluate and treat patients, develop plans of care, and coordinate with a multidisciplinary team to support independence and quality of life for residents.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted license as Physical Therapist in the state of practice.
• Current Live CPR Certification.
• Two-step TB skin test required.
• Proof of Auto Insurance.
• Familiarity with assisted living, independent living, geriatrics and/or home health.
• Demonstrates knowledge of evidence based therapy interventions.
• Communicates effectively and professionally in verbal and written interactions.
• Ability to lift 50 pounds.
• Duties may involve lifting up to 100 pounds with safety procedures.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess effectiveness of treatment plans and participate in staffing meetings.
• Perform discharge evaluations and provide safety recommendations and education resources.
• Communicate patient needs and progress to the treatment team, physicians, patients and families.
• Supervision of Physical Therapy Assistants as needed.
• Collaborate with other disciplines to coordinate care and improve outcomes.
