Job summary

BrightSpring Health Services is seeking a Physical Therapist for the Wilson Assisted Living site. This PRN role involves evaluating patients, creating and adjusting personalized treatment plans, and coordinating with a care team to optimize outcomes.

You will provide one-on-one care, participate in family conferences, and contribute to a flexible schedule designed to balance patient needs and clinician availability.

Qualifications

• Minimum bachelors degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapist license in the practicing state.
• CPR certification current; TB skin test up-to-date.
• Auto insurance proof and ability to lift 50 lbs; up to 100 lbs with safety.
• Familiar with geriatrics, assisted living or home health settings.

Responsibilities

• Perform evaluations and develop treatment plans for patients.
• Regularly reassess treatment effectiveness and adjust plans.
• Discharge evaluations with safety recommendations and referrals.
• Communicate patient needs and progress to care team and family.
• Supervise Physical Therapy Assistants as needed.
• Interdisciplinary collaboration to improve patient outcomes.
