Job summary

Adoration Health is expanding and hiring a licensed Physical Therapist to deliver one-on-one care in a patient-focused setting. A $5,000 sign-on bonus is offered and the role supports flexible, autonomous scheduling in various care environments, including Independent Living, Assisted Living, and outpatient services.

Responsibilities include evaluations, treatment planning, re-assessments, and coordination with physicians and families while supervising PTAs as needed.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Current, unrestricted Physical Therapist license required.
• Current CPR certification.
• Two-step TB test required.
• Proof of auto insurance required.
• Demonstrates knowledge of evidence-based therapy techniques.
• Familiarity with assisted living, independent living, geriatrics and/or home health.
• Strong communication skills in verbal and written form.
• Able to lift 50 pounds and assist with transfers up to 100 pounds.

Responsibilities

• Perform evaluations and develop treatment plans.
• Re-assess treatment effectiveness and attend staff/family conferences.
• Conduct discharge evaluations and provide safety recommendations.
• Communicate patient needs and progress to the treatment team and family.
• Supervise Physical Therapy Assistants as needed.
• Collaborate with other disciplines to improve patient outcomes.
