Job summary

AlignMed Partners seeks a Full Time Advanced Practice Provider (NP or PA) for Birch Healthcare Center in Rochester, NH. This on-site role offers a base salary range of $112,320 to $131,040 per year and includes a no-cap incentive program.

New grads welcome; no on-call or weekends. The APC will provide primary and urgent care to nursing center residents, collaborating with the Medical Director and Center Leadership to deliver compassionate, high-quality care.

Qualifications

• Graduate from an accredited School of Nursing program with RN license and NP certification (MSN) in Older Adult or Family NP.
• PA candidates must graduate from an ARC-PA accredited program and pass PANCE; PA licensure and NCCPA certification required.
• Minimum two years of experience with adult or geriatric populations preferred.

Responsibilities

• Provide primary and urgent care services to nursing center residents.
• Collaborate with Center Leadership and Medical Director to meet goals and priorities.
