Job summary

AlignMed Partners is seeking a Full Time Advanced Practice Provider (NP or PA) for Birch Healthcare Center in Rochester, NH. This role earns $112,320–$131,040 per year and includes an incentive bonus program. New grads are welcome; 40 hours/week, no on-call or weekend requirements.

The team supports post-acute and long-term care with strong clinical leadership, mentorship, and career advancement opportunities. On-site collaboration with leadership ensures high-quality resident care.

Qualifications

• Graduate from accredited nursing program; RN licensed; NP with Masters in nursing (Older Adult or Family NP).
• PA must graduate from ARC-PA accredited program and pass PANCE; PA licensure and DEA where applicable.
• Two years of adult/geriatric care experience preferred.

Responsibilities

• Provide primary and urgent care to nursing center residents on site.
• Collaborate with Center Leadership and Medical Director on patient care.
• Support AlignMed Partners’ mission with high-quality care and compassion.
