AlignMed Partners is seeking a Full Time Advanced Practice Provider (NP or PA) in Epsom, NH. No weekends or on-call. New grads welcome. Base salary $116,480–$128,960 with incentive bonus. Two years geriatric experience preferred; licensure and national certs required.

The AP Provider collaborates with leadership and medical staff to deliver primary/urgent care to nursing center residents, aligned with our mission and values.

Qualifications

• Graduate from an accredited School of Nursing program with RN license and NP certification.
• Nurse Practitioner or PA track with required national certs and licensure.
• Minimum two years experience with adult/geriatric populations preferred.

Responsibilities

• Provide primary and urgent care to nursing center residents.
• Collaborate with center leadership and medical director for patient care.
• Deliver high-quality health care with compassion in a post-acute/long-term care setting.
