Job summary

Cynet Health is seeking a Physician Assistant specialized in Internal Medicine to care for geriatric patients in an outpatient clinic and in the community. The role includes coordinating with an interprofessional team to ensure comprehensive elder care.

The position supports both full-time and part-time schedules, with travel to patients' homes as needed and managed through an EMR system. High standards of patient safety and quality are expected.

Qualifications

• Active license required
• BLS certification required
• Board Certification or Board Eligible
• DEA required
• Geriatric experience preferred

Responsibilities

• Provide care predominantly to geriatric patients in outpatient clinic and during home visits.
• Collaborate with an interdisciplinary team of 11 professionals to coordinate patient care.
• Manage patient encounters using an electronic medical record (EMR) system.
• Typically 4–6 patient encounters per shift with no on-call duties.
• Travel to patients' residences, including assisted living facilities or personal homes, when needed.
