Qualifications

• MD or DO from an accredited medical school.
• 3+ years of outpatient primary care experience.
• Active Florida medical license and current DEA registration.
• Specialization in Internal Medicine, Family Medicine, or Geriatric Medicine.

Responsibilities

• Provide comprehensive primary care services for a geriatric population.
• Deliver preventive care, wellness visits, and chronic disease management.
• Develop individualized care plans for geriatric patients.
• Collaborate with nurses, care coordinators, and support staff to ensure seamless care.
• Utilize EHR to document encounters and track patient outcomes.
• Engage in quality improvement initiatives.
• Educate patients and families on prevention and healthy living.
