Total Care Therapy seeks a licensed Occupational Therapist to join our clinician-owned practice serving assisted living and independent living communities in Tipp City, OH. You will evaluate residents and create personalized plans to improve daily living activities, cognitive function, and safety.

Qualifications

• Experience working with older adults in geriatrics, home health, assisted living, or skilled nursing is preferred but not required.
• Proficiency in evaluating residents and developing individualized treatment plans.
• Strong documentation and communication with residents, caregivers, and staff.

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise COTAs in accordance with state regulations.
• Complete timely and accurate documentation using the EMR.
• Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
