Qualifications

• Licensed OT in the state of practice
• Graduate of an accredited Occupational Therapist program
• Interest or experience in skilled nursing or geriatric rehabilitation
• Passion for patient-centered care and improving quality of life for older adults

Responsibilities

• Evaluate patients and develop personalized care plans focused on functional independence, safety, and quality of life
• Deliver care through therapeutic activities, ADL training, and adaptive techniques
• Help patients perform daily tasks such as dressing, bathing, grooming, and meal preparation
• Partner with interdisciplinary teams, caregivers, and nursing staff to support patient outcomes and continuity of care
• Track patient progress, maintain accurate clinical documentation, and adapt treatment plans as needed
