Job summary

Blue Stone Therapy is seeking an Occupational Therapy Assistant (COTA) to provide rehabilitation services to adult and geriatric patients at Stoney Point Meadows in Cedar Rapids, IA. You will work under the supervision of a licensed Occupational Therapist to implement treatment plans aimed at improving ADLs, mobility, strength, cognition, and independence.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Current OTA license or eligibility for licensure in the applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Strong communication, organizational, and clinical reasoning skills.
• Commitment to patient-centered care and professional standards.

Responsibilities

• Provide skilled occupational therapy treatment in accordance with established plans of care.
• Implement individualized treatment programs focused on functional and measurable outcomes.
• Treat adult and geriatric patients in a skilled nursing facility (SNF) setting and rehabilitation settings.
• Monitor patient response to treatment and communicate progress with the supervising Occupational Therapist.
• Maintain timely, accurate documentation to support quality care and reimbursement standards.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.
