Job summary

Blue Stone Therapy in Ossian, IA is seeking a full-time or part-time Occupational Therapy Assistant (COTA) to provide rehabilitation services in a SNF setting. You will implement treatment plans under the supervision of a licensed OT, focusing on ADLs, mobility, cognition, and independence for adult and geriatric patients.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Current OTA license (or eligibility) in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Strong communication, organization, and clinical reasoning skills.
• Commitment to patient-centered care and professional standards.

Responsibilities

• Provide skilled occupational therapy treatment in accordance with established plans of care.
• Implement individualized treatment programs focused on functional outcomes.
• Treat adult and geriatric patients in SNF and rehab settings.
• Monitor patient response and communicate progress with supervising OT.
• Maintain timely documentation to support care and reimbursement standards.
• Collaborate with interdisciplinary team to support positive outcomes.
