Job summary

Blue Stone Therapy is seeking an Occupational Therapy Assistant (COTA) to provide rehabilitation services to adult and geriatric patients at Crestview Specialty Care. The COTA works under the supervision of a licensed Occupational Therapist to implement plans designed to improve ADLs, mobility, strength, cognition and independence.

Qualifications

• Graduate of an accredited OTA program and eligible for licensure.
• Experience in skilled nursing, long-term care or post-acute rehab preferred.
• Strong communication, organization and clinical reasoning skills.
• Patient-centered care and professional standards commitment.

Responsibilities

• Provide skilled occupational therapy treatment per plans of care.
• Implement individualized programs focused on functional outcomes.
• Treat adult and geriatric patients in SNF and rehab settings.
• Monitor patient response and communicate progress to supervising OT.
• Maintain timely documentation supporting quality care and reimbursement.
• Collaborate with interdisciplinary team and facility staff.
