Job summary

The Chatham Hills Subacute Care Center is seeking a dedicated Occupational Therapist to join our rehabilitation team. You will assess referred patients, develop and oversee treatment plans, and document progress while educating patients and families in therapy methods.

Collaborative care with other disciplines is essential. Ideal candidates have an accredited OT degree, active state licensure, and experience in long‑term or geriatric care, with strong documentation and professional conduct.

Qualifications

• Graduated from an accredited Occupational Therapist program.
• Must hold a valid OT license and state registration.
• Experience supervising Occupational Therapy Assistants.
• Long-term care or geriatric experience preferred.
• Strong documentation writing skills.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care to evaluate medical condition, function, and rehab potential.
• Establish and administer treatment plans with goals tailored to patient capacity and tolerance.
• Document patient status and progress; educate patients and families in therapy approaches.
• Collaborate with other disciplines to provide patient-centered care.
• Maintain confidentiality, HIPAA compliance and professional standards.
