Job summary

The Chatham Hills Subacute Care Center is seeking an Occupational Therapist to join our growing rehabilitation team. The OT will assess referred patients, establish and administer treatment plans, educate patients and families, and document patient progress under physician direction.

The role requires license and work with geriatric populations, with duties including supervising OTAs and coordinating with the interdisciplinary team to deliver patient-centered care.

Qualifications

• Graduated from an accredited Occupational Therapist program.
• Must hold a valid professional license and state registration.
• Experience with long-term/geriatric care preferred.
• Strong documentation and discharge planning skills.

Responsibilities

• Assess referred patients and develop rehabilitation plans with goals.
• Provide direct patient care and monitor progress and tolerance.
• Educate patients and families on therapy methods.
• Oversee Occupational Therapy Assistants and document weekly notes.
• Collaborate with the care team to ensure patient-centered treatment.
