Job summary

The Chatham Hills Subacute Care Center is seeking an Occupational Therapist to join our rehab team. You will assess referred patients to determine functional status and rehabilitation potential, and develop individualized treatment plans under physician direction.

Responsibilities include documenting progress, educating patients and families, supervising OT assistants, coordinating with other disciplines, and ensuring compliance with confidentiality and residents' rights.

Qualifications

• Must have graduated from an ACOTE-accredited OT program.
• Hold a valid OT license in the state of practice.
• Experience supervising OT assistants is preferred.

Responsibilities

• Assess referred patients to determine rehabilitation needs and potential.
• Develop and administer individualized treatment plans with goals.
• Document patient condition and progress, including discharge planning.
• Educate patients and families in therapy techniques.
