Job summary

Chatham Hills Subacute Care Center is seeking an Occupational Therapist to join our rehabilitation team. The role involves assessing referred patients, developing and supervising individualized treatment plans, and delivering direct patient care in a long-term geriatric setting.

The OT will establish goals, document progress, and educate patients and families on therapy methods while collaborating with a multidisciplinary team to ensure optimal outcomes.

Qualifications

• Graduate from an accredited OT program.
• Must hold a valid state OT license/registration.
• Experience in long‑term care and/or geriatric care preferred.
• Strong documentation and record-keeping skills.
• Ability to develop and supervise comprehensive treatment plans.

Responsibilities

• Assess referred patients to determine rehabilitation needs and potential.
• Provide direct patient care and assess medical condition, functional capabilities and limitations.
• Establish and administer treatment programs with specific goals.
• Set realistic, measurable goals and document patient progress.
• Educate patients and families on therapy approaches and expected outcomes.
• Collaborate with all disciplines to provide best patient-centered care.
• Supervise Occupational Therapy Assistants and ensure quality care.
• Maintain confidentiality and adhere to HIPAA and resident rights.
