Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Active state license as an Occupational Therapist.
• Experience with geriatrics in skilled nursing or post-acute settings preferred.
• Ability to develop individualized treatment plans and document progress.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.
• Document in EMR and participate in interdisciplinary team rounds.
• Educate families and caregivers about care plans and home safety.
