Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Active Illinois OT license or eligibility
• Experience caring for seniors in SNF/post-acute settings
• Strong EMR documentation and record-keeping skills

Responsabilités

• Evaluate residents to determine functional status
• Develop individualized treatment plans
• Deliver evidence-based interventions to improve ADLs, mobility, cognition and safety
• Document care in EMR and maintain accurate records
