Job summary

Renewal Rehab seeks a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Must hold a current OT license in Ohio.
• Experience working with seniors in SNF or post-acute settings preferred.
• Strong assessment and treatment planning skills.

Responsibilities

• Evaluate residents to determine OT needs and goals.
• Develop individualized treatment plans and modify as needed.
• Deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.
