Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifikationen

• Licensed Occupational Therapist with active license in the U.S.
• Ability to evaluate residents in skilled nursing and post-acute facilities.
• Develop individualized treatment plans based on assessment findings.
• Deliver evidence-based interventions to improve ADLs, mobility, cognition and safety.
• Document in EMR and communicate with families and the care team.
