Stellar Health Group is seeking an experienced Occupational Therapist to join our rehabilitation team, delivering high-quality care to residents in a supportive, patient-centered environment.
The OT will collaborate with an interdisciplinary team, evaluate patients, develop individualized treatment plans, educate families, and ensure timely documentation and discharge planning in accordance with Medicare and facility requirements. Massachusetts licensed clinicians only.

Provide occupational therapy services that promote excellence in rehabilitation care, interdisciplinary cooperation, and quality patient outcomes.
Promote the facility's geriatric rehabilitation program to medical professionals and the community.
Evaluate patients in a timely manner according to state, payer, and facility requirements.
Screen patients upon admission and whenever requested by the Director.
Develop individualized treatment plans and appropriate goals based on patient evaluations.
Provide treatment according to the established plan of care.
Communicate and collaborate with other disciplines and facility staff as indicated by the treatment plan.
Participate in family conferences when requested.
Educate and counsel patients, families, and staff regarding the rehabilitation process as appropriate.
Complete routine documentation in accordance with Medicare, facility, and other payer guidelines.
Record and submit all required documentation and treatment charges accurately and in a timely manner.
Participate in discharge planning and interdisciplinary discussions as needed.
Current Massachusetts state license as an Occupational Therapist.
Ability to work collaboratively as part of a dynamic and cooperative interdisciplinary team.
Flexibility to distribute workload according to departmental needs and individual responsibilities.
Strong evaluation and assessment skills.
Ability to develop individualized treatment plans and goals.
Ability to participate in consultations, teaching, and meetings with residents, staff, families, and other healthcare professionals.
Strong communication and interpersonal skills.
Ability to educate and counsel patients and staff regarding occupational therapy and rehabilitation needs.
