Job Summary
Stellar Health Group is seeking a dedicated and compassionate Occupational Therapist (OT) to join our energetic and collaborative therapy team.

Responsibilities
- Provide occupational therapy services that promote excellence in rehabilitation care, interdisciplinary cooperation, and quality patient outcomes.
- Promote the facility's geriatric rehabilitation program to medical professionals and the community.
- Evaluate patients in a timely manner according to state, payer, and facility requirements.
- Screen patients upon admission and whenever requested by the Director.
- Develop individualized treatment plans and appropriate goals based on patient evaluations.
- Provide treatment according to the established plan of care.
- Communicate and collaborate with other disciplines and facility staff as indicated by the treatment plan.
- Participate in family conferences when requested.
- Educate and counsel patients, families, and staff regarding the rehabilitation process as appropriate.
- Complete routine documentation in accordance with Medicare, facility, and other payer guidelines.
- Record and submit all required documentation and treatment charges accurately and in a timely manner.
- Participate in discharge planning and interdisciplinary discussions as needed.
Qualifications
- Current Massachusetts state license as an Occupational Therapist.
- Ability to work collaboratively as part of a dynamic and cooperative interdisciplinary team.
- Flexibility to distribute workload according to departmental needs and individual responsibilities.
- Strong evaluation and assessment skills.
- Ability to develop individualized treatment plans and goals.
- Ability to participate in consultations, teaching, and meetings with residents, staff, families, and other healthcare professionals.
- Strong communication and interpersonal skills.
- Ability to educate and counsel patients and staff regarding occupational therapy and rehabilitation needs.
