Job summary

Oasis Therapy Services in Burr Ridge, IL is seeking an Occupational Therapist to provide rehabilitation services in a skilled nursing facility on a PRN basis. The role focuses on activities of daily living, mobility, safety, and independence.

You will evaluate residents, develop treatment plans, educate residents and families, supervise assistants, and ensure thorough Net Health documentation within an interdisciplinary care team. Flexible scheduling and competitive compensation are offered.

Qualifications

• Active IL OT license required.
• Graduate of an accredited OT program.
• Current CPR certification.
• Geriatric or skilled nursing rehab experience preferred.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide treatment focused on activities of daily living, functional mobility, safety, and independence.
• Monitor residents’ progress and modify treatment plans as needed.
• Complete discharge planning and provide recommendations for continued care.
• Educate residents, families, and caregivers on exercises, adaptive techniques, and equipment.
• Supervise Occupational Therapy Assistants when assigned.
• Complete accurate Net Health documentation and collaborate with the interdisciplinary care team.
