Job summary

Oasis Therapy Services in Burr Ridge, IL is seeking an Occupational Therapist to provide rehabilitation services in a skilled nursing facility. This PRN role uses Net Health EMR and offers flexible scheduling with growth opportunities.

You will evaluate residents, develop individualized treatment plans, educate residents and families on ADLs, monitor progress, and collaborate with the care team to ensure safe, effective care. Illinois license and CPR are required.

Qualifications

• Active and current Illinois Occupational Therapist license.
• Graduate of an accredited Occupational Therapy program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric, skilled nursing, or rehabilitation experience is preferred.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide treatment focused on activities of daily living, functional mobility, safety, and independence.
• Monitor residents' progress and modify treatment plans as needed.
• Complete discharge planning and provide recommendations for continued care.
• Educate residents, families, and caregivers on exercises, adaptive techniques, and equipment.
• Supervise Occupational Therapy Assistants when assigned.
• Complete accurate Net Health documentation and collaborate with the interdisciplinary care team.
