Job summary

Renewal Rehab is seeking a licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed Occupational Therapist authorized to practice in the relevant state.
• Experience delivering OT services to seniors in skilled nursing or post-acute settings.

Responsibilities

• Evaluate residents in skilled nursing and post-acute facilities.
• Develop individualized treatment plans and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.
