Job summary

Renewal Rehab seeks a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop treatment plans, and deliver interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers. Renewal Rehab offers a supportive culture, manageable caseloads, mentorship and growth

Qualifications

• Licensed OT in good standing.
• Experience providing occupational therapy for seniors in SNF/post-acute settings preferred.
• Ability to document clearly in EMR and communicate with families.

Responsibilities

• Evaluate residents in skilled nursing and post-acute facilities.
• Develop individualized treatment plans and deliver evidence-based interventions.
• Improve ADLs, mobility, cognition, and safety.
• Document in EMR and track progress.
