Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. Onsite role, you will evaluate residents and develop individualized treatment plans.

You will deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety, document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Active Illinois OT license required.
• Experience with seniors in SNF or post-acute settings preferred.
• Ability to assess ADLs, mobility, cognition and safety.

Responsibilities
