Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role evaluates residents, develops individualized treatment plans, and delivers evidence-based interventions to improve adl, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed OT with active state license in Illinois.
• Experience in skilled nursing or post-acute settings preferred.
• Ability to develop and implement individualized treatment plans.

Responsibilities

• Evaluate residents and determine treatment needs.
• Develop and monitor individualized treatment plans to improve ADLs, mobility, cognition, and safety.
• Deliver evidence-based interventions.
• Document in EMR and maintain accurate records.
• Collaborate with an interdisciplinary team.
• Educate families and caregivers about care plans.
