Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, creating individualized treatment plans, and delivering evidence-based interventions to enhance ADLs, mobility, cognition, and safety.

You will document in an EMR, collaborate with an interdisciplinary team, and educate families and caregivers to support outcomes and quality of life for seniors.
