Job summary

Renewal Rehab is seeking a licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Active IL OT license or eligibility.
• Experience caring for seniors in SNF/post-acute settings.
• Ability to document clearly in EMR.
