Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role evaluates residents, develops individualized treatment plans, and delivers evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Must hold a current OT license in the state.
• Experience in geriatric or skilled nursing settings preferred.
• Evaluate residents and develop individualized treatment plans.
• Deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.
• Document in EMR and collaborate with an interdisciplinary team.

Responsibilities

• Perform initial and ongoing assessments of residents' functional abilities.
• Create and adjust individualized treatment plans.
• Deliver evidence-based OT services to improve independence in daily activities, mobility, cognition, and safety.
• Educate families and caregivers on care plans and home programs.
