Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist for an on-site role to provide patient-centered care for seniors in skilled nursing and post-acute facilities. You will evaluate residents, create individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

Documentation in EMR, collaboration with a diverse team, and caregiver education are required.

Qualifications

• OT license in good standing
• Experience delivering patient-centered OT care preferred
• Ability to assess ADLs, mobility, cognition and safety

Responsibilities

• Evaluate residents in skilled nursing and post-acute settings
• Develop individualized treatment plans based on assessment findings
• Deliver evidence-based interventions to improve ADLs, mobility, cognition and safety
• Document in EMR system and maintain accurate records
