Qualifications

• Ability to treat participants and manage complex medical problems.
• Knowledge of state and federal laws pertaining to patient care.
• Experience with frail or elderly population and supervisory experience preferred.

Responsibilities

• Conducts initial assessments of participants both at intake and quarterly.
• Develops and implements therapeutic Plans of Care.
• Manages the primary care clinic and evaluates participants' health.
