Job summary

Vitae Health Systems is seeking a dedicated Nurse Practitioner to provide patient-centered medical care to residents in skilled nursing facilities. You will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve outcomes.

Qualifications

• Master’s degree in Nursing from an accredited NP program.
• Active and unrestricted APN license in the respective state.
• DEA registration and prescriptive authority (or eligibility).
• Board certification as a Nurse Practitioner (e.g., FNP, AGNP).
• 2 years of clinical experience preferred, ideally in geriatrics or long-term care.
• Strong clinical assessment, diagnostic, and treatment planning skills.
• Knowledge of chronic disease management and geriatric care principles.
• Ability to deliver value-based care focused on outcomes and cost-efficiency.

Responsibilities

• Perform comprehensive health assessments and manage acute and chronic conditions.
• Order and interpret diagnostics, prescribe treatments, and review medications.
• Deliver timely clinical interventions to respond to patient status changes and reduce hospitalizations.
• Support value-based care initiatives by focusing on quality outcomes and data-guided decisions.
• Collaborate with physicians and care teams to ensure coordinated, efficient care and participate in care plan meetings.
• Facilitate smooth transitions of care across settings and ensure continuity of treatment.
• Educate residents and families on diagnoses, treatment plans, and preventive care strategies.
• Maintain accurate, timely, and compliant documentation.
