Job summary

Vitae Health Systems seeks a dedicated Nurse Practitioner to provide high-quality, patient-centered care to residents in a skilled nursing facility. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life.

The role offers a full-time exempt position with a compensation range of $110,000-$130,000, plus the opportunity to earn bonuses.

Qualifications

• Master’s degree in Nursing required.
• Active APN license in respective state required.
• DEA registration and prescriptive authority (or eligibility).
• Board certification as a Nurse Practitioner (e.g., FNP, AGNP).

Responsibilities

• Perform comprehensive health assessments and manage acute and chronic conditions.
• Provide ongoing medical management including diagnostics, treatments, and medication reviews.
• Deliver timely clinical interventions to improve outcomes and reduce hospitalizations.
• Collaborate with physicians and interdisciplinary team members for coordinated care.
• Facilitate care transitions and participate in quality improvement efforts.
