Job summary

GuideStar Eldercare seeks a Nurse Practitioner to address dementia and neurocognitive needs in nursing homes and assisted living facilities across Eastern Kentucky. You will operate with private practice autonomy, supported by corporate clinical leadership and in-house admin teams.

You will travel daily, perform initial evaluations, cognitive testing, and ongoing therapy services, and document care while building strong facility relationships.

Qualifications

• State licensure as a Nurse Practitioner and certification.
• Master’s degree in NP specialty from an accredited university.
• Certification through ANCC or AANP.
• 1+ year clinical APRN experience preferred; geriatrics/long-term care preferred.

Responsibilities

• Travel daily to assigned facilities from home (Eastern Kentucky).Provide initial evaluations, therapy services, and cognitive testing.
• Document the need and extent of services for each patient.
• Create and maintain positive professional relationships with facilities, patients, and families.
