Job summary

Guidestar Eldercare is seeking a Nurse Practitioner to address the needs of residents with dementia, Alzheimer’s, and other neurocognitive disorders in nursing homes and assisted living facilities. You will experience the autonomy of private practice, supported by corporate leadership and in-house administrative teams.

Qualifications

Active California NP license.
Pass pre-employment background check and drug screening.
Complete credentialing process.
Vaccination against COVID-19 and flu as required by policy.

Responsibilities

Travel daily to assigned facilities from home.
Provide initial evaluations, continuing therapy services, and cognitive testing.
Document the need and extent of services for each patient.
Create and maintain positive professional relationships with client facilities, patients, and families.
