Guidestar Eldercare is seeking a Nurse Practitioner to address dementia and neurocognitive disorders in nursing homes and assisted living facilities. You will work autonomously with a defined caseload under corporate clinical leadership and in-house administrative support.

As part of a multidisciplinary team focused on safety, function, and dignity, you will travel daily from home to facilities, perform initial evaluations, ongoing therapy, and cognitive testing, and document care needs for each

Qualifications

• Proof of State NP licensure.
• Pass pre-employment background check and drug screening.
• Complete credentialing process.
• Vaccination against COVID-19 and flu is required.

Responsibilities

• Travel daily to assigned facilities from your home.
• Provide initial evaluations and continuing therapy services.
• Conduct cognitive testing.
• Document the need and extent of services for each patient.
• Build positive professional relationships with facilities, patients, and families.
