Job summary

Geisinger LIFE Wilkes-Barre is seeking an experienced Nurse Practitioner to join its aging population program in a days shift. The position emphasizes senior care within a team-based model and requires patient-centered, evidence-based practice.

The NP will support acute visits, post-hospital follow-ups, and annual assessments, collaborate with physicians, pharmacists, therapists, and care coordinators, and document care in the EMR. A Master's degree and NP certification are required.

Qualifications

• Master's degree in nursing with NP credential required.
• BLS certification and NP license required; PA RN license helpful.
• Experience with elderly, nursing home level of care preferred.

Responsibilities

• Provide acute and post-hospital follow-ups and manage care plans.
• Collaborate with physicians, pharmacists, therapists, and care coordinators.
• Order, interpret tests, prescribe therapies per evidence-based practice.
• Document in EMR accurately and maintain patient safety and education.
