Job summary

Senior LIFE PACE in Greensburg, PA is seeking a full-time CRNP to assess the healthcare of chronically ill geriatric members in collaboration with physicians and interdisciplinary teams.

This hands-on role supports improving quality of life and independence at home and in centers, with a daylight schedule Monday–Friday, competitive pay, comprehensive benefits, PTO and holidays, CME, and malpractice coverage. Requires PA CRNP and 1+ year geriatric experience.

Qualifications

• Must be a certified CRNP with Pennsylvania license.
• Experience with chronically ill geriatric populations preferred.
• Willing to participate in continuing education/workshops to maintain license.
• Strong collaboration with physicians and multidisciplinary teams.

Responsibilities

• Take health histories and perform in-depth exams for new members to determine care plans.
• Initiate, evaluate and review medical orders; identify opportunities to improve care.
• Carry out procedures and document all care and recommendations.
• Order, interpret and follow up on lab tests and results.
• Coordinate consultations with in-house and external providers including physicians and therapists.
• Educate staff, members, caregivers and families to promote independence at home and in centers.
• Conduct ongoing assessments to adjust Plans of Care based on feedback and evaluations.
