Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to provide primary and preventive care to residents in long-term care facilities in Kentucky. You will assess, diagnose, and manage chronic conditions, coordinate with an interdisciplinary team, and support families and facility staff.

The role emphasizes geriatric care, treatment planning, and adherence to Kentucky laws for prescribing medications, with opportunities for professional growth and completing continuing education credits.

Qualifications

Current, unrestricted NP license in Kentucky.
Board certification in family, adult, or geriatrics preferred.
DEA licensure required for prescribing controlled substances.
Experience in long-term care settings is highly desirable.
Proficient with electronic medical records (EMR).
Ability to manage patients with multiple chronic conditions within an interdisciplinary team.

Responsibilities

Provide primary and preventive care for residents in skilled nursing facilities.
Manage residents with acute changes in condition and treatment planning.
Engage in advanced care planning and chronic care management.
Prescribe medications in accordance with Kentucky law.
Order, review, and follow up on treatments and diagnostics.
Collaborate with nursing home staff and primary care physicians.
