Job summary

CareMed is seeking a Nurse Practitioner to deliver primary and preventive care to residents in long-term care facilities in Kentucky. The provider will manage acute changes, plan advanced care, and coordinate treatments with facility staff and the primary physician.

The role requires an unrestricted Kentucky NP license, board certification, and DEA licensure, with experience in geriatrics or adult care and proficiency with EMR systems.

Qualifications

• Current, unrestricted NP license in Kentucky.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team.

Responsibilities

• Provide primary and preventative care for long term care patients in skilled nursing facilities.
• Manage residents with acute change in condition.
• Advance care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with KY law.
• Order, review and follow up with treatments and diagnostic testing.
• Communicate with nursing home staff on changes in condition and care plan.
• Collaborate with primary care physician.
